Important Information About Your  2025 FORM 1095-B

Integrated Solutions, Inc. will not automatically mail Form 1095-B to individuals covered under our health plan for the 2025 tax year unless a copy is requested.

Form 1095-B provides information about your health coverage and may be used when filing your federal income tax return. However, the IRS does not require you to submit Form 1095-B with your tax return.

If you would like to receive a copy of your Form 1095-B, you may request one using any of the methods below:

Requests must include your full name, mailing address, and the last four digits of your Social Security Number to verify your identity.  Upon receipt of your request, a copy of your Form 1095-B will be furnished within 30 days.
